This page is for school homeless liaisons to make a referral to Move Forward Counseling, LLC.
How to Make a Referral to The Bridge Project in PA
The following two forms must be submitted before scheduling an appointment.
- Please fill out and submit the following referral form below. For help with the form or technical issues, please write to info@moveforwardpa.com.
- Please download and complete the Referral Intake Form with the necessary information.
Note on Age
- If the client is under 14, a parent or legal guardian must complete and sign the intake forms.
- If the client is 14 or older, they may sign the intake forms themselves.
Where to Submit Forms
Once the referral (below) and intake forms are both completed, please email them to info@moveforwardpa.com.
Please reach out with any questions to 717-462-7003 option 1. There might be additional forms to sign depending on who the client schedules with. Thank you for helping us ensure students can be placed with a therapist as quickly as possible.
